A. No. With LifePerks for Health there are no claims to file and no forms to fill out. LifePerks for Health provides deep discounts on health services without the red tape.
Q. How do I use my LifePerks for Health program?
A. It's easy. To locate a participating provider, simply refer to your Member Booklet, go to the provider locator on the website or call our toll free number 1-866-347-9821. If the service requires an appointment with a provider, simply call one of our Customer Care Specialists who will assist you with provider selection and answer any questions you may have. For all other benefits, simply present your card at your point of service and receive your discount.
Q. Can anyone in my immediate family use my LifePerks for Health Benefits Savings?
A. Yes. If you select the Family Plan, family members residing in your household and dependent away from home (for example, children at college) can use any of your LifePerks for Health benefits. We've got your entire family covered.
Q. What if I have a pre-existing condition?
A. LifePerks for Health eliminates ALL pre-existing condition limitations regardless of your current health. We will not refuse our discount benefits to any member with a pre-existing condition.
Q. Will I need to fill out any complicated claim forms?
A. No. With LifePerks for Health there are no forms or paperwork whatsoever. There are no exclusions and there is never a waiting period for services and products.
Q. Does the Pharmacy benefit apply to brand name medications?
A. Yes. Discounts apply to both brand name and generic prescription drugs as well as non-prescription drugs.
Q. How much can I save and are there any limits on how often I can use my benefits?
A. Discounts on products and services range from 5%-60%. Refer to your Member Booklet to find specific discounts. There are no maximums or limits on how often you can use your benefits.
